What Is Acne?
Acne is an ongoing inflammatory disorder of the skin, caused by pores in hair follicles getting blocked with excess oil, sloughed-off skin cells, and bacteria. Although it's typically thought of as a teenage issue due to hormone changes, it can affect anyone at any age, including young people navigating puberty or adults grappling with stress-triggered flare-ups and hormonal shifts.
The condition develops through a multi-step process: sebaceous glands attached to hair follicles produce sebum (oil) that normally travels up the follicle to the skin surface. When this process malfunctions, whether from excess oil production, abnormal shedding of follicle cells, or bacterial proliferation, the follicle becomes blocked, creating an environment where Cutibacterium acnes (C. acnes) bacteria thrive. This bacterial overgrowth triggers inflammation, resulting in the red, swollen, sometimes painful lesions we recognize as acne.
Acne comes in various forms: comedones (blackheads and whiteheads), papules (small red bumps), pustules (pimples with visible pus), nodules (large, painful lumps beneath the skin), and cysts (deep, pus-filled lesions). The severity and type of acne determine appropriate treatment approaches and influence scarring risk. Acne is a medical condition, not simply a cosmetic nuisance or result of poor hygiene.

Types of Acne Scars: Understanding Your Specific Concern
Acne scars develop when inflammatory acne lesions (particularly deep cysts and nodules) damage the skin's dermal structure. When inflammation extends deep into the dermis, it destroys collagen and other structural proteins within the skin. The body attempts to repair this damage, but the healing process often produces disorganized collagen rather than the organized matrix of healthy skin.
Atrophic scars form when insufficient collagen is produced during the healing process, resulting in depressions. Hypertrophic and keloid scars result from excessive collagen production.
Atrophic Scars (Depressed/Indented): These represent the majority of acne scars and result from loss of tissue during the healing process.
Ice Pick Scars: Narrow, deep, sharply defined scars resembling small punctures. These are the deepest scar type, extending into the dermis like an inverted cone.
Rolling Scars: Wider depressions with sloped, wave-like edges. Caused by fibrous bands tethering the dermis to deeper structures, creating surface depressions. These give skin a textured, uneven appearance.
Boxcar Scars: Round or oval-shaped dips in the skin with clear, sharp edges. Can be shallow or deep, wider than ice pick scars, and commonly appear on the temples and cheeks.
Hypertrophic and Keloid Scars (Raised): These raised, thickened scars result from excessive collagen production during the healing process. Hypertrophic scars don't extend beyond the original wound area, while keloid scars extend beyond, creating larger, raised formations. More common in people with darker skin tones and often appear on the chest, back, and jawline.
Individual scarring tendency depends on genetics, inflammation severity, depth of the acne lesion, and whether the lesion was manipulated (picked or squeezed), which worsens tissue damage and increases scarring risk significantly.
